    To acknowledge receipt of the substantive change request. To include the relocation of the additional location from 2390 SR 848, New Milford, PA 18834 to 420 Tioga West Plaza, Tunkhannock, PA 18657 within the institution’s scope of accreditation. To require immediate notification when instruction commences at the new location and of the date that instruction ceases at the former location. To note that the Commission reserves the right to rescind approval of this substantive change if any developments reveal additional information that might have affected the Commission’s decision and/or the requested substantive change is not implemented within one calendar year from the date of this action. The next evaluation visit is scheduled for 2023-2024.

The Commission requires written notification within thirty days of the commencement of operations at this additional location as well as the closure of the relocated location.

To include the additional location at Lackawanna College Sunbury Center, North Sunbury Street, Sunbury, PA 17801 within the scope of the institution’s accreditation. The Commission requires written notification within thirty days of the commencement of operations at this additional location. Operations at the additional location must commence within one calendar year from the date of this action. To note that the monitoring report has been received and will be acted on by the Commission at the June meeting. The next evaluation visit is scheduled for 2023-2024.

    To acknowledge receipt of the substantive change request and to approve the relocation of the Towanda Center additional location from 1 Elizabeth Street, Suite 2, Towanda, PA 18848 to 1024 South Main Street, Towanda, PA 18848 and to include the new additional location within the scope of the institution’s accreditation. The Commission requires written notification within 30 days of the commencement of operations at this additional location and the closure of the other location. In the event that operations at the additional location do not commence within one calendar year from the approval of this action, approval will lapse. To remind the institution of the request for a monitoring report, due April 1, 2017, documenting further implementation of (1) a well-defined system of collegial governance, including written policies outlining governance responsibilities of administration and faculty which are readily available to the campus community and the periodic assessment of the effectiveness of institutional leadership and governance (Standard 4); (2) fair and impartial processes, published and widely available, to address student grievances, such as alleged violations of institutional policies (Standard 6); (3) ongoing assessment of student success, including but not limited to retention, that evaluates the match between the attributes of admitted students and the institution's mission and programs, and reflects its findings in admissions, remediation, and other related policies (Standard 8); and (4) student support services including (a) athletic programs that are regulated by the same academic, fiscal, and administrative principles, norms, and procedures that govern other institutional programs and (b) ongoing assessment of student support services and the utilization of assessment results for improvement (Standard 9). To further remind of a request that the institution inform the Commission of any change in its status regarding continued participation in Title IV HEA programs. The Periodic Review Report is due June 1, 2020.
