To acknowledge receipt of the substantive change request. To include the additional location at State Correctional Institution, Dallas PA, 1000 Follies Road, Dallas, PA 18612 within the institution’s scope of accreditation. To require immediate notification when instruction commences at the additional location. To note that the Commission may rescind this action if instruction does not commence within one calendar year from the date of this action. The next evaluation visit is scheduled for 2027-2028. - October 29, 2019
To acknowledge receipt of the substantive change request. To include the reclassification of the other instructional site at Tobyhanna Army Depot, 11 Hap Arnold Blvd, Tobyhanna, PA 18466 to an additional location within the institution’s scope of accreditation. To require immediate notification when the change is implemented. To note that the Commission may rescind this action if the requested substantive change is not implemented within one calendar year from the date of this action. The next evaluation visit is scheduled for 2027-2028. - June 27, 2019

To request that, beginning in 2020 and in conjunction with each Annual Institutional Update prior to the Mid-Point Peer Review in 2024, the institution provide further evidence of periodic assessment of the effectiveness of assessment processes utilized by the institution for the improvement of educational effectiveness (Standard V). The Commission requires written notification within thirty days of the commencement of operations as an additional location. To approve the reclassification of the instructional site at Geisinger Medical Center, 100 North Academy Avenue, Danville, PA 17822 as an additional location and to include the location provisionally within the scope of the institution's accreditation, pending a site visit within six months of the commencement of operations at this location. To remind the institution of the request for a monitoring report, due April 1, 2016, documenting further implementation of an organized and sustained assessment process for all programs, including general education, with evidence that results are used to improve teaching and learning (Standard 14). To remind the institution that its accreditation may be in jeopardy because of insufficient evidence that the institution is currently in compliance with Standard 14 (Assessment of Student Learning). To note that the institution remains accredited while on warning. To note that the due date for the next evaluation visit will be established when accreditation is reaffirmed. - March 6, 2014
