To request a monitoring report, due April 1, 2016, further documenting an organized, systematic and sustainable process to assess the achievement of student learning goals in all programs, including in general education, that foster student learning and development and that uses results to improve teaching and learning (Standard 14). The Periodic Review Report is due June 1, 2018.

To request a progress report, due April 1, 2015, documenting further evidence of (1) planning and improvement processes that are clearly communicated, provide for constituent participation, and incorporate the use of assessment results in budgeting and planning (Standard 2); (2) institutional administrative structures that facilitate learning and foster quality improvement and that qualified administrative leaders are in place appropriate to the goals, type, size and complexity of the institution and who have the skills and time necessary to discharge their duties effectively (Standard 5); (3) steps taken to assure continuity and stability of institutional administration (Standard 5); and, (4) implementation of a documented, organized, and sustained process to assess student learning and institutional effectiveness (Standards 7 and 14). The Periodic Review Report is due June 1, 2018.
