    To note that the substantive change visit to the additional location at 250 Lincoln Avenue, Lockport, NY 14094 which occurred on June 10, 2021, and the information provided in the substantive change request was verified. To note that the institution hosted a virtual site visit in lieu of an on-site visit in accordance with the United States Department of Education (USDE) guidelines published March 17, 2020. To note that a verification visit is required by USDE guidelines and will be conducted within a reasonable period of time following the virtual site visit.

To request a monitoring report, due April 1, 2018, documenting further progress on the implementation of (1) goals and objectives or strategies, both institution-wide and for individual units that are clearly stated, reflect conclusions drawn from assessment results, are linked to mission and goal achievement, and are used for planning and resource allocation at the institutional and unit levels (Standard 2); and (2) a documented, organized, and sustained process to assess achievement of expected student learning outcomes in all programs, with evidence that assessment information is used to improve teaching and learning (Standard 14). The date for the next accreditation review will be determined by the Commission when it revises the accreditation cycle.
