To request a monitoring report, due October 1, 2009, documenting further progress in addressing the following: (1) financial concerns previously identified by the Commission (Standard 3); (2) Board/executive leadership (Standards 4 and 5); (3) the existence and exercise of appropriate oversight and control over the institution and its activities (Standards 3 and 5); and (4) development/implementation of a sustainable climate and culture of institutional integrity (Standard 6). To place the institution on probation, and to request a monitoring report, due by October 15, 2006, documenting how the institution has addressed: (1) financial concerns previously identified, (2) stabilization and reformation of the institution's Board and executive leadership, and (3) appropriate oversight and control over the institution and its activities. A visit will follow submission of the report. The Commission acted further to direct a staff visit for the purpose of meeting with the Federal Monitor who has been selected by the Office of the United States Attorney for the District of New Jersey, and institutional representatives to discuss the Commission's expectations. The Commission notes that it is taking these actions because of continuing concerns following the April 2006 special visit, the publication of subsequent reports by the Monitor, and because of the continuing lack of audited financial statements for FY05.
