    To acknowledge receipt of the substantive change request. To include the additional location at 37 Lane Avenue LaVale, MD 21502 within the institution's scope of accreditation. To require immediate notification when instruction commences at the additional location. To note that the Commission may rescind this action if instruction does not commence within one calendar year from the date of this action. The next evaluation visit is scheduled for 2022-2023.

    To accept the monitoring report and to note that a small team visit will not be conducted. To request that, beginning in 2019 and in conjunction with each Annual Institutional Update, the institution provide further evidence of (1) the development and implementation of organized and systematic assessment processes that evaluate the extent of student achievement and documents the use of assessment results to improve educational effectiveness (Standard V) and (2) clearly stated institutional and unit-level objectives that are linked to mission and reflect conclusions drawn from assessment results (Standard VI). The next evaluation visit is scheduled for 2022-2023.

    To acknowledge receipt of the substantive change request. To include the reclassification of the branch campus at Somerset County Campus, 6022 Glades Pike, Suite 100, Somerset, PA 15501-4300 as an instructional site and to include the change within the scope of the institution's accreditation. To remind the institution of the Commission's request of November 17, 2016 for a monitoring report due March 1, 2018. The next evaluation visit is scheduled for 2022-2023.

    To accept the monitoring report and to note the visit by the Commission's representatives. To remove the warning because the institution is now in compliance with Standard 2 (Planning, Resource Allocation, and Institutional Improvement) and Standard 7 (Institutional Assessment) and to reaffirm accreditation. To request a monitoring report, due March 1, 2018, documenting further implementation of (1) goals and objectives or strategies, both institution-wide and for individual units, that are clearly stated, reflect conclusions drawn from assessment results, are linked to mission and goal achievement, and are used for planning and resource allocation at the institutional and unit levels (Standard 2); (2) a documented, organized, and sustained institutional effectiveness assessment process, with evidence that assessment information is used to improve programs, services, and processes and is shared and discussed with appropriate constituents (Standard 7); and (3) an organized, systematic and sustainable process to assess the achievement of student learning goals in all programs, including general education, that fosters student learning and development and that uses results to improve teaching and learning (Standard 14). A small team visit may follow submission of the monitoring report. The next evaluation visit is scheduled for 2022-2023.

To continue to warn the institution that its accreditation may be in jeopardy because of insufficient evidence that it is in compliance with Standard 2 (Planning, Resources, and Institutional Improvement) and Standard 7 (Institutional Assessment). To note that the institution remains accredited while on warning. To request a monitoring report, due September 1, 2016, documenting evidence that the institution has achieved and can sustain compliance with Standards 2 and 7, including but not limited to evidence of a documented, organized, and sustained institutional effectiveness assessment process, with evidence that assessment information is used to improve programs, services, and processes and is shared and discussed with appropriate constituents (Standards 2 and 7). A small team visit will follow submission of the monitoring report. The date of the next accreditation review will be established when accreditation is reaffirmed.

To remind the institution of the Commission's action of June 25, 2015, warning the institution that its accreditation may be in jeopardy because of insufficient evidence that it is currently in compliance with Standard 2 (Planning, Resource Allocation and Institutional Renewal), Standard 7 (Institutional Assessment), and Standard 14 (Assessment of Student Learning). To note that the institution remains accredited while on warning. To remind the institution of the monitoring report due March 1, 2016. The due date for the next Periodic Review Report will be established when accreditation is reaffirmed.

    To warn the institution that its accreditation may be in jeopardy because of insufficient evidence that the institution is currently in compliance with Standard 2 (Planning, Resource Allocation and Institutional Renewal), Standard 7 (Institutional Assessment), and Standard 14 (Assessment of Student Learning). To note that the institution remains accredited while on warning. To request a monitoring report, due March 1, 2016, documenting evidence that the institution has achieved and can sustain compliance with Standards 2, 7, and 14, including but not limited to evidence of the development and implementation of (1) goals and objectives, both institution-wide and for individual units, that are clearly stated, reflect conclusions drawn from assessment results, are linked to mission and goal achievement, and are used for planning and resource allocation at the institution and unit levels (Standard 2); (2) a documented, organized, and sustained institutional effectiveness assessment process, with evidence that assessment information is used to improve programs, services, and processes and is shared and discussed with appropriate constituents (Standard 7); and (3) an organized, systematic and sustainable process to assess the achievement of student learning goals in all programs, including general education, that fosters student learning and development and that uses results to improve teaching and learning (Standard 14). In addition, to request that the monitoring report document an ongoing program of appropriate orientation, training, and support for faculty participating in electronically delivered offerings (Standard 13). A small team visit will follow submission of the monitoring report. To direct a prompt liaison guidance visit to discuss Commission expectations. The date for the Periodic Review Report will be established when accreditation is reaffirmed.
